1. Tax Form: All UCRs must turn in W9 Form before the start of the program. International students need to turn in a w8ben Form.

2. Travel Reimbursement Form: Upon your arrival, UCRs must complete an SFI reimbursement form and return it to Carla with original receipts. Carla will help you with this on your first day at SFI.

3. International Students: Upon your arrival, you will need to show all your documents to SFI's visa officer. Carla will help you with this as well.
